Instructions
Please use this form to re-apply for re-admittance to the college after extended leave, withdrawal or disqualification. Upon submittal of the form, your petition will be reviewed, and you will be contacted regarding your re-admittance or any required next steps. The recommended deadline for the petition is 30 days prior to the start of the next term. Petitions received within 7 days of the start of the term will be considered for the following term (i.e. the term after the upcoming term).

The readmission committee will carefully review your previous student record and your responses below as part of the readmission decision process. Please provide clear and detailed answers that fully explain your circumstances and your plan for ensuring success in the program.
Acknowledgment

I understand that I have submitted a request for a petition for re-admittance to PCHS. I understand that this request is subject to review and approval by the appropriate academic authorities. Review for the petition for re-admittance may take between 5-7 business days.

By submitting this form, I confirm that:

  1. The information provided in this form is accurate and complete to the best of my knowledge.
  2. All holds whether academic or financial will need to be cleared before re-admission review.
  3. I understand that re-admission is subject to approval based on academic criteria, program availability, and any other relevant considerations.
  4. I am aware that any changes to my program may impact my academic progress, graduation timeline, and financial aid eligibility.
  5. I will adhere to all policies and procedures related to re-admission as outlined by PCHS.
  6. I understand that this acknowledgment does not guarantee re-admission to PCHS.
  7. Students that have been inactive for more than one year from their previous program at PCHS need to go through the admissions department.
